A new set of paperwork must be turned in for Fall 2012 season.  We cannot transfer paperwork from a prior school year.

PAPERWORK GUIDELINES FOR REGISTERING FOR WMSABC SPORTS!

ALL PAPERWORK MUST BE TURNED IN AT SAME TIME...do not submit until you have ALL required pieces included.  For Fall 2012 sports, you MUST turn in a full set of paperwork, we do not transfer paperwork from the prior school year!
 
Incomplete paperwork will not be accepted.
 
*Paperwork must be in a sealed envelope. On the outside of envelope you must have the athletes first and last name and the sport they are trying out for this spring season.
 
*Paperwork is to be turned in to the school office. There will be a locked box for paperwork to be placed.

*You must sign the log in sheet that will be with the security box the day you place your paperwork in box. Please do not interrupt office staff with booster club questions. If you are unsure about something then contact a board member via email.

The following Items are required prior to July 16, 2012 registration deadline unless prior arrangements have been made with the WMS Booster Club

Physical Form (signed by doctor)  - must be dated AFTER August 5, 2011

  1. Insurance Waiver Form (NOTARIZED)
  2. Copy of Insurance Card (front and back)
  3. Completed & Signed  Sports Concussion Form
  4. Complete Online Registration for Sport (See "Register Online" option on left-side of www.wmsabc.com

CHEERLEADING KICKOFF MEETING

On Tuesday, May 29th at 6PM, there will be a kick off meeting for rising 6th, 7th and 8th grade cheerleaders and their parents.  This meeting will discuss the tryout/camp dates (July 30-Aug 3), the tryout process (including new requirements by the county), optional clinics on June 18th and July 16th.  Coaches for each grade level will be introduced at this time.
